UCI Departments of Neurological Surgery & Neurology Neurotrauma Conferences

Objective(s): 1 Review current evidence and guidelines regarding the use of repeat head CT in patients with traumatic brain injury, including factors that predict radiographic progression and influence clinical decision-making. 2 Discuss the impact of anticoagulant and antiplatelet therapies on intracranial hemorrhage progression in TBI patients, and evaluate contemporary strategies for rapid reversal and hemostatic optimization. 3 Compare different institutional and specialty-specific approaches to monitoring, imaging, and managing TBI in anticoagulated patients, highlighting areas of consensus and ongoing controversy. 4 Examine the role of patient characteristics such as age, comorbidities (e.g., cirrhosis), mechanism of injury, and neurologic examination in guiding individualized imaging schedules, reversal protocols, and repeat CT utilization; this will allow us to discuss the BIG guidelines. 5 Determine when repeat imaging meaningfully changes management, when it does not, and how multidisciplinary collaboration can improve outcomes in complex neurotrauma patients.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Review current evidence and guidelines regarding the use of repeat head CT in patients with traumatic brain injury, including factors that predict radiographic progression and influence clinical decision-making.
  2. Discuss the impact of anticoagulant and antiplatelet therapies on intracranial hemorrhage progression in TBI patients, and evaluate contemporary strategies for rapid reversal and hemostatic optimization.
  3. Compare different institutional and specialty-specific approaches to monitoring, imaging, and managing TBI in anticoagulated patients, highlighting areas of consensus and ongoing controversy.
