This educational activity provides a discussion of the Hyperlink 1, 2 & 3 trials examining home blood pressure telemonitoring and pharmacist care to improve hypertension control. A presentation of the work and results of the San Diego Wellness Collaborative. An examination of deploying community health workers in primary care for better outcomes. A review of demonstration projects showing better outcomes by adding pharmacists to the care team, including the Los Angeles Barbershop Blood Pressure Study. The live lectures for this recording took place on May 21, 2020, and September 11, 2023.

You must complete all required content and post-tests to receive CME credit. To obtain a CME certificate, return to My CME and complete the assigned evaluation under Evaluations and Certificates. 

For participants that need Maintenance of Certification, be sure to complete the MOC attestation in the Course Content once you have completed all required elements.

MOC Approval Statement (ABIM Only):
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.75 Medical Knowledge MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

1. Explain how deploying pharmacists on the care team & telemonitoring can significantly improve blood pressure and cholesterol control, while yielding significant cost savings.
2. List the benefits of and implementation steps for deploying community health care workers as part of the care team.
